Reporting Requirements | Youth Central

As set out in the FReeZA 2016 -2018 Guidelines and Conditions of Service Delivery, it is a requirement that FReeZA organisations provide the Community and Economic Participation Branch with two reports per year that details the outcomes from your FReeZA events, training activities and committee participation.

FReeZA organisations will also be required to provide the following forms and reports:

  • FReeZA Event Notices to be lodged at least three weeks before an event
  • First Aid and security incidents (where an incident has occurred).
  • Risk Management and Occupational Health and Safety Plans (for outdoor events).

All completed forms, unless otherwise indicated below, should be emailed and/or posted to the Community and Economic Participation Branch as follows:

To promote your event through the FReeZA Gig Guide and helps us to gather your event information an Event Notice Form should be lodged at least three weeks before the event.

To lodge your Event Notice Form:
1. Click on the link to the
2. Save it to your computer
3. Fill in the details of your event according to the instructions on the Form
4. Save the Form and attach it to an email
5. Email the completed Form to freeza@dpc.vic.gov.au

FReeZA 2016-2018 Incident Report (248 KB)
To report a security incident, a first aid incident or any other unusual occurrence during any FReeZA event. You must report any incident to the Office for Youth within one week of the incident.
